How to Register

 

We welcome requests for registration from patients living in or moving to Great, Upper and Lower Cambourne. You will be registered with one of the General Practitioners, but you can see any of the doctors in the practice.

Registering is easy and you can do it in a number of ways:

· If you have your NHS card – simply sign it, fill in your new Cambourne address and telephone number (landline and mobile) and send the card to us or bring it into the surgery. Please complete and return a new patient questionnaire for each adult and a child registration form for each child.

· If you don’t have your NHS card – come into the surgery and fill in a NHS registration form and new patient questionnaire or child registration forms or download them from the link below and post them to us. Please remember to sign form GMS1 at the bottom.

If you are taking regular medication you will be invited to make an appointment with our Clinical Pharmacist. This will ensure that your repeat prescriptions are set up and you can get your medication without delay when you need it.

If you have not been registered with a GP in this country before we will need additional information before we are able to register you. Please telephone us or come into the surgery for more information.

Summary Care Record

What is the Summary Care Record?

Summary Care Record (SCR) is an electronic record of important information about a patient’s health. The summary care record is uploaded from General Practice clinical systems subject to practice agreement and subject to a patient not opting out.

What information is included in the Summary Care Record?

It will initially have information about current medications, allergies and any adverse reactions to medication. Additional information may be added over time.

You can choose whether or not you want your record uploaded from our clinical system onto the Summary Care Record.  If you do not want your details uploaded please come into the surgery and complete an opt out form or you can download the form from www.nhscarerecords.nhs.uk.  This website also provides more information about the Summary Care Record.

If you are a visitor to Cambourne needing an emergency appointment please telephone us on 01954 282153.

Emergency treatment is free to everyone, regardless of nationality or immigration status. If the doctor does not consider your condition to be an emergency and you are not entitled to NHS treatment you will be charged for the consultation and any prescription that is issued to you.
